Obtaining Your Medical Records
The Health Information Services (HIS) department at Beaufort Memorial fulfills requests for information for all inpatient, outpatient and clinical services. The HIS staff is happy to assist you in obtaining your medical records upon receipt of a valid authorization. You can mail, fax or personally deliver your authorization to release health information.
You can also access your medical records from the comfort and privacy of your home or office using our secure patient portals.
Electronic Request for Records
Beaufort Memorial now offers an Online Patient Record Request tool for the following individuals to request patient records:
- Patients requesting their own records
- Parents of minor patients requesting records
- Caregivers acting on behalf of a patient (i.e. Power of Attorney)
The Online tool verifies your identity by asking for a photo of your driver's license, which can be taken via webcam or smartphone. If the patient is deceased, please make sure to attach a copy of the death certificate. There is no additional charge to use this service beyond fees outlined below.
