Patients and families are part of the TEAM
that helps develop a comprehensive treatment program including
a Multidisciplinary Plan of Care that addresses, but is not
limited to, medical management, self-care skills, functional
mobility, nutrition, pain and medication management, wound
care, bowel and bladder management, education, family conferences
and discharge planning.
