Our unique Bridge to Therapy Program eases the transition from hospital-based rehabilitation to any of our three convenient outpatient clinics. Through the program, physical therapists visit patients at home several times during the first week of recovery. This important in-home rehabilitation, which is customized to individual needs, often streamlines the transition to outpatient therapy and gets patients on the road to a more effective outcome with greater mobility, faster.
